This is great in so many ways. First, I have had other gel packs, and I have found them hard to put on and hold in place. This one has an elastic velcro band that allows you to make it tight enough to stay on while you work, or even eat. I use this for tennis elbow, and the club pro said best thing to do is use warmth before I play, then follow with cold after playing. This is PERFECT. I put the gel pack in the microwave for 40 sec [you have to check the strength of your microwave, then test it before burning yourself]. Here's another great thing: when you put the gel pack in the sleeve, it seals on the end, also with velcro. My last one did not have that, so the pack ended up burning my arm a little. This helps prevent that. When I am done with the warmth, before I leave for tennis, I put the pack in the freezer. When I get back 2 hours later, take it out, and use if for the cold application. 2 for 1. Also, have found both heat and cold stays longer with this pack. Another thing that impresses me with this Adalid is that I have gotten three personalized emails from Jonah, the president. That's awesome. Mostly, I was being thanked for choosing Adalid, but also got an update on the progress of the shipment. Actually, came sooner than Jonah suggested it would.
One more little trick that I learned: it's not so easy to wrap this tight around the area below my elbow; if my wife helps, it's a piece of cake, but I discovered the easiest way to do it myself: remember that there is a good deal of stretch in the velcro strap, so I simply create the wrap by creating the circle BEFORE I put it on my arm; you can play with it until you get the right amount of tension. Then, simply put your arm through the circle you have created [it's like putting on an armband]. Because there is stretch, once you get it right, you will get a snug fit, without having to wrestle with it, trying to wrap it around your arm. Truly love this product.
3/27/15 - UPDATE: Still loving this, as it's so easy and effective. Definitely helping with my tennis elbow.
This product has saved my knees! I walk a few miles a day and recently added running, but was plagued by "runner's knee." Several hours after running, my knees would be hot to the touch and very sore when used (i.e., going from sitting to standing). I ordered these and use them immediately after running. The difference has been a game-changer for me and I'm excited to not have to give up running.
These are well-made. I keep them in the freezer and they do not freeze to a state of solidity. They remain flexible, yet hold their cold temperature for over an hour. I find them a tad cold to be in direct contact with my skin, so I simply use a paper towel between the gel pack and my knee. The cushioned straps mean that I don't have to sit while I "ice" my knees, and can be productive while taking care of myself.
The gel pack fits nicely into the holder which is quite comfortable and adjusts well.
The only negative is that the gel pack gets so stiff that I need to microwave it to
soften it so that it will fit around my shoulder. I once had a gel pack that stayed
flexible when frozen. It would be great if this one did that. Then I'd give it a five.
Just got the new gel pack which is flexible, even after sitting in the freezer all night.
I've been using it on a strained knee and I couldn't ask for better.

Love it! I'm using this for tendonitis and it provides a lot of relief from the constant burning and pain. I wear it for 30 minutes at a time and, when the ice pack warms up, swap it with another insert I already had. The only way I would recommend improving it would be if it had a pouch that snaps to hold the gel pack, and perhaps a buckle which would make it easier to tighten. I have to set the velcro to the approximate size of my elbow, insert the gel pack, and then slip it over my hand to my elbow. Of course, this wasn't designed specifically for an elbow either. Good quality and a good value...

Its compact size also makes it ideal to travel with. As for the length of time to heat it in the microwave, I found 45-50 seconds to be just right in my machine. Any longer might result in a leak or even explosion, so please DO read the manufacturer's instructions and watch it in the microwave the very first time you heat it. Most highly recommended.
